Aging, exposure to environmental stressors, lifestyle choices, and genetics can all contribute to the way our facial appearance changes over time. As we get older, our face loses collagen, elastin, fat, and bone. Unwanted wrinkles, volume loss, and sagging of the skin become more prominent. Whether you’re hoping to restore a more youthful, smoother appearance or enhance specific features, dermal fillers at Bryn Mawr’s Cirillo Cosmetic Dermatology Spa are versatile treatments that can help you sculpt and refresh your face. They not only soften wrinkles and creases, but can also add volume and lift to enhance your appearance.

What Are Dermal Fillers?
What are dermal fillers made of? How do dermal fillers work?
Injectable gels or soft tissue fillers are synthetically made from natural substances. They are injected into the face to soften wrinkles and creases, restore volume, and lift sagging skin. Dermal filler injections are a safe, effective, minimally invasive treatment with a goal of creating a smoother, younger-looking appearance.
The Cirillo Cosmetic team uses dermal fillers to plump lips, soften upper lip lines, restore fullness to cheeks, fill in facial creases and hollows, diminish scars, erase fine lines, rejuvenate hands, lift the brow, jowls, or a downturned “unhappy” mouth, and replenish dark hollows and tear troughs under the eyes. Dermal fillers can also be used “off label” (the FDA did not approve these uses) to treat the neck, décolleté, abdomen, knees, arms, and stretch marks.

Who Is a Good Candidate for Dermal Fillers?
There are many types of dermal fillers available in 2023, so suitability can vary depending on each patient’s specific needs. But who is the ideal candidate for dermal fillers? In most cases, patients who are physically and mentally healthy, non-smokers, have realistic expectations, and are committed to maintaining healthy skin will be good candidates for dermal fillers.
Ideal candidates for hyaluronic acid dermal fillers will usually include those who are experiencing early signs of aging, such as lines around the nose and mouth. In many cases, the best candidates for biostimulatory fillers such as Radiesse® (calcium hydroxylapatite or CaHA) and Sculptra® (poly-L-lactic acid or PLLA) will be those over the age of 40.
Keep in mind that several factors affect the duration of results, including the body’s rate of collagen production, lifestyle, skin type, age, and muscle activity in the area that is being treated.

What Are the Different Types of Dermal Fillers?
Not all dermal fillers are the same, and there are many different types available. Although you may not know which type of filler you need, it can be helpful to understand the basics.
Fillers fall into two main categories: hyaluronic acid and biostimulatory fillers, such as Radiesse® (calcium hydroxylapatite or CaHA) and Sculptra® (poly-L-lactic acid or PLLA).
Hyaluronic acid fillers, which are the most popular option, are temporary and reversible treatments that usually last for 12 to 18 months. They can be a good choice for patients who are new to fillers.
Although biostimulatory injections can add some instant volume, the main purpose of these dermal fillers is to provide longer-term smoothing and plumping by stimulating the body’s natural production of collagen and elastin. They are better for more volume-depleted areas and for deeper injections, but cannot be reversed or dissolved like hyaluronic acid fillers.
There are also different products within each family of filler, with formulations of various particle sizes and viscosities used for different purposes.
Hyaluronic Acid Fillers
Hyaluronic acid is a humectant that naturally occurs in the skin and can retain 1,000 times its weight in water. The body produces this molecule to keep the skin moisturized. Hyaluronic acid dermal fillers include Juvéderm®, Restylane®, Belotero®, and RHA® (Resilient Hyaluronic Acid) fillers.
Calcium Hydroxylapatite (CaHA)
Radiesse® is a filler that contains a mineral-like substance found in human bones called calcium hydroxyapatite (CaHa). It is ideal for addressing deeper wrinkles and is injected into bony areas (cheeks, jawline) and the hands as it camouflages unwanted veins and tendons well. Radiesse® stimulates the production of collagen and elastin.
Poly-L-lactic Acid Filler (PLLA)
Sculptra® is made up of poly-L-lactic acid (PLLA) particles, which promote collagen production over two to three months. PLLA is also used to make sutures. Sculptra® is commonly used to treat volume loss in the temples and mid-face.

What Is Recovery Like After Dermal Fillers?
Many patients are curious about what to expect after having dermal fillers, with questions such as “How long is recovery after dermal fillers?” and “What should I do and not do?”
When administered by an experienced provider, dermal fillers provide natural-looking results with minimal side effects. A topical anesthetic may be used prior to treatment, depending on the type of dermal filler chosen and the area being treated.
After treatment, most patients resume normal activity within one day. Your Cirillo Cosmetic injector will provide you with pre- and post-procedure instructions to ensure the best outcome from your treatment with injectable fillers.
Temporary side effects from facial fillers may include redness, swelling, tenderness, and occasional bruising at the injection sites. Most side effects clear within five to 10 days. Cirillo Cosmetic injectors are very experienced in injecting dermal fillers with soft, flexible microcannulas. The use of microcannulas greatly reduces the risks of bruising, swelling, and discomfort.
How Long Do the Results of Dermal Fillers Last?
Most facial fillers offer temporary improvement of the treated area and require repeat treatments to maintain the desired result. Dermal fillers can last between 12 to 36 months depending upon which filler is used. Smart lifestyle choices can also prolong the results of fillers (e.g. wearing sunscreen daily, not smoking).
Why Choose Cirillo Cosmetic Dermatology Spa for Dermal Fillers?
Although dermal fillers are one of the most popular cosmetic dermatology treatments available, they are most safely injected by an experienced, medically credentialed provider. When fillers are administered by inexperienced injectors, the incidence of serious side effects increases dramatically.
At Cirillo Cosmetic, our founding dermatologist, Victoria A. Cirillo-Hyland, MD, is a nationally recognized expert in the use of dermal fillers. Highly skilled and knowledgeable about the use of these products, Dr. Cirillo-Hyland has earned the “Circle of Excellence in Soft-tissue Fillers Award” from the American Society for Dermatologic Surgery.
Bryn Mawr, Newtown Square, and Delaware, Montgomery, and Chester County patients choose our Main Line practice for our 55+ years of experience. We help patients maintain a youthful, natural appearance with safe, conservative treatments.
Which Options Are Available Aside from Dermal Fillers?
Dermal fillers are primarily used for static wrinkles, which remain on the face when at rest. For expression wrinkles/”wrinkles in motion,” we recommend neuromodulators like BOTOX®, Dysport®, and DAXXIFY®. Dermal fillers are frequently used in combination with each other or with the neuromodulators to create a refreshed, more youthful appearance in what is referred to as a “liquid facelift,” without the cost, risk, or recovery time of facelift surgery.
